I think the problem is due to customization. Basically, I upgraded my startwr pack to an Origin 325a I bought and installed ingame components I died, and logged off. I customized the 325a on the RSI web-site (I did NOT use paid upgrades, such as component packages) I logged back in and put an insurance claim on the destroyed ship.
